We are NOW enrolling for 2011-2012 School Year!! 

For your convenience, we have suppplied the enrollment forms that you will need to have completed to reserve your child's placement in our Preschool program.  Below there are five (5) links to open and print, altogether there are 8 pages to the enrolllment process.  

In order to print these forms, you'll need to set your margins to print; 0.7 for left and right margin, and 0.5 for top and bottom margin. 

We encourage parent involvement in our classroom, so we ask that you print out the volunteer statement form and turn that in with your enrollment forms.  You'll find the link below.

Click here for the "Enrollment forms". Please complete this form in its entirety, it contains 3 pages.  It has some personal information about your preschool that will allow us to know them better.  Thanks :)

Click here for the "student medical report form". Please fill out the top of this form, it contains 2 pages; and then take this to your primary care physician and have them complete the medical update portion.  You'll also need to include a copy of your child's immunization records with this as well. 

Click here for "release of photography and permission to travel".  This is simply a brief coverage allowing us to take pictures of your preschooler and publish them on the website, it contains 1 page.  It will also give us permission to go on field trips, although you will still sign an individual permission form for all trips.  Thanks!

Click here for "student Emergency medical information form".  This is contains the emergency contacts in case you are not available for medical consent; it contains 1 page.  This needs to be completed in its entirety as well. Thank you.

Click here for "Volunteer Statement".  This form allows you to join the class as a volunteer and we strive to have all our parents involved in their child's education therefore, we'd like to have one of these from every parent.  This is very basic; it contains 1 page, but it's extremely important.  Please fill it out and sign it.  Thanks.
